In order to conduct its business, EnsureMePh Insurance Broker, Inc. (InsuranceKo) needs to collect and use certain information (“Personal Data”) about individuals (“Data Subjects”).

This document, describes how Personal Data must be collected, handled and stored in line with our commitment to protect the privacy, confidentiality and security of the personal data of our Data Subjects and  in compliance with the Data Privacy Act of 2012 (DPA) and its Implementing Rules and Regulations.

“We”, “Us”, “Our” refers to EnsureMePh Insurance Broker, Inc. operating under the brand name InsuranceKo. “You”, “customer”, “client” refers to the Data Subjects including users of the InsuranceKo website.

 

1. Collection of Personal Information

We collect and process your personal information strictly in accordance with the Data Privacy Act of 2012, and for the specific purposes that these information were provided. The types of personal data that we may typically collect, process, and retain may include:

1.      Personal Identification and contact information to provide products and services such as, but not limited to, full name, date of birth, age, gender, civil status, contact number, family and dependents’ details, address, email address, and occupation.

2.      Financial and payment information

3.      Official Identification documents such as government-issued ids, birth certificate, and marriage certificate;

4.      Medical and health information

We collect your personal and sensitive personal data when you register, sign-up or use our products and services or contact us about them. We receive personal information supplied to us via our website, emails, telephone calls and any electronic interfaces. We also collect through organization/s which you authorize to share your data with us.

 

2. Use of Personal Information

Personal data collected shall be used to facilitate your transaction needs when availing of our products and services, or in the execution of our obligations with Parties or Personal Information Controllers (PIC) that we work with such as for policy administration, underwriting and claims processing for the insured customers; for documentation purposes; or for compliance to regulations. We collect, process and store your personal data in accordance with the terms of the specific notice provided to you when taking the services provided.

 

3. Storage Retention and Destruction

We will ensure that personal data under our custody are protected against any accidental or unlawful destruction, alteration and disclosure as well as against any other unlawful processing. We will implement appropriate security measures in storing collected personal information, depending on the nature of the information. All information gathered shall not be retained for a period longer than five (5) years or for a period stated in the data sharing and cooperation agreements with our clients and partners or as required by regulations.  After the period specified in our agreements with clients and partners and as required by regulations, all hard and soft copies of personal information shall be disposed and destroyed, through secured means.\

 

4. Access

Due to the sensitive and confidential nature of the personal data under our custody, only the client and our authorized representatives shall be allowed to access such personal data, for any purpose, except for those contrary to law, public policy, public order or morals. Authorization to access data shall be dependent on the role of the InsuranceKo personnel.

Any request from customer to amend personal data shall be processed in accordance with the guidelines issued by InsuranceKo or the PIC that we work with – this process shall include verification of identity of the requesting customer. 

 

5. Disclosure and Sharing

All employees and personnel of InsuranceKo shall maintain the confidentiality and secrecy of all personal data that come to their knowledge and possession, even after resignation, termination of contract, or other contractual relations. Personal data under the custody of InsuranceKo shall be disclosed only pursuant to a lawful purpose, and to authorized recipients of such data. Disclosure and sharing of data shall be in accordance with the Data Sharing Agreements or Outsourcing Agreements with InsuranceKo’s partners wherein InsuranceKo may act as both the Receiving party and Disclosing party.

 

6. Consent and Legitimate Use

By providing us with your personal data, including where applicable sensitive personal data, you consent to the collection and use of any Personal Information you provide in accordance with the above purposes and this privacy statement. Notwithstanding the foregoing, there may be separate legitimate grounds for InsuranceKo to process or store your Personal Information.

 

7. Direct Promotion

We may use your information for us to send out campaigns of our products and services we hope you find interesting, relevant, and that would suit your needs. You have the right to withdraw your consent or unsubscribe from receiving any promotion or marketing communications.
